𝘏𝘰𝘸 𝘰𝘯𝘦 𝘭𝘦𝘢𝘥𝘦𝘳’𝘴 𝘣𝘰𝘭𝘥 𝘷𝘪𝘴𝘪𝘰𝘯 𝘳𝘦𝘴𝘩𝘢𝘱𝘦𝘥 𝘩𝘦𝘢𝘭𝘵𝘩𝘤𝘢𝘳𝘦 𝘢𝘤𝘤𝘦𝘴𝘴 𝘪𝘯 𝘚𝘰𝘶𝘵𝘩 𝘊𝘰𝘵𝘢𝘣𝘢𝘵𝘰 𝘢𝘯𝘥 𝘪𝘯𝘴𝘱𝘪𝘳𝘦𝘥 𝘳𝘦𝘧𝘰𝘳𝘮𝘴 𝘣𝘦𝘺𝘰𝘯𝘥 𝘱𝘳𝘰𝘷𝘪𝘯𝘤𝘪𝘢𝘭 𝘣𝘰𝘳𝘥𝘦𝘳𝘴
When Reynaldo S. Tamayo Jr. took his oath as governor of South Cotabato on June 30, 2019, many expected a change in local governance. What few foresaw was a major overhaul of how healthcare is delivered in an entire province — transforming a system once burdened by cost barriers into one where care comes before cashiers.
On his very first day in office, Governor Tamayo implemented a bold initiative: zero-balance billing in all provincial government-run hospitals across the province. This meant no more billing counters, no upfront payments for patients, and — in the words of the people — no need to queue at a cashier just to receive care.
“Palagi ko sinasabi na bayad na ito eh — yung mga Doctor at Nurses, may pasyente o wala — sweldado na ‘yan.” Governor Tamayo, explaining the principle behind cashierless hospital services.
Governor Tamayo’s cashierless model shifted the financial responsibility of hospitalization from patients to the provincial government. Salaries of doctors, nurses, utility workers, and other staff are shouldered by the provincial government, allowing residents and even non-resident patients to receive medical services without paying fees at hospital doors.
This initiative went beyond political rhetoric — it has real impact on families who might otherwise struggle to cover medical bills. Thousands of patients have benefited from free hospitalization, and the program has also been linked to expanded free healthcare services like subsidized dialysis treatment for indigent patients.
𝙄𝙣𝙨𝙩𝙞𝙩𝙪𝙩𝙞𝙤𝙣𝙖𝙡𝙞𝙯𝙞𝙣𝙜 𝙁𝙧𝙚𝙚 𝙃𝙚𝙖𝙡𝙩𝙝𝙘𝙖𝙧𝙚
Recognizing the need to sustain these reforms beyond his term, Governor Tamayo worked with the Sangguniang Panlalawigan to institutionalize the zero-billing system through formal ordinances. This legislative effort ensures that free hospitalization is not merely a policy of the moment, but a structural part of provincial governance.
Governor Tamayo didn’t stop at policy alone. Under his leadership, the province is expanding healthcare infrastructure, securing funds for major hospital expansions, and initiating the construction of new hospitals in Tantangan and Koronadal City to decongest existing facilities and bring care closer to communities.

South Cotabato’s cashierless hospital model drew praise from public officials outside the province. The mayor of Medellin, Cebu publicly hailed Governor Tamayo as the “Father of Cashierless Hospitals in the Philippines,” noting his leadership inspired local governments to explore similar free hospitalization reforms.
𝙍𝙚𝙘𝙤𝙜𝙣𝙞𝙩𝙞𝙤𝙣 𝘽𝙚𝙮𝙤𝙣𝙙 𝙃𝙚𝙖𝙡𝙩𝙝𝙘𝙖𝙧𝙚
Governor Tamayo’s people-centered leadership has also been recognized nationally. In late 2025, the Climate Change Commission honored him as a “Climate Hero” for integrating social protection programs like free medical care with broader goals of community resilience and climate readiness.

As Governor Tamayo continues his third term, his healthcare reforms remain at the forefront of his governance legacy — a legacy rooted in the belief that healthcare is a right, not a privilege. For many residents of South Cotabato, the simple act of entering a hospital without financial worry symbolizes more than policy — it represents dignity, compassion, and a leader who chose service over self-interest.
